Purpose. Performers, interviewees, extras and location owners complete this form to give Studio Aurora and its client permission to record and use their image, voice, performance or property in a production.

1.When to use this form

A release must be signed by every identifiable person who appears in a recording and by the owner or manager of every private location used, before the first take. A guardian must sign for anyone under 18. No release means the material cannot be used.

2.What the person is agreeing to

The release grants permission to record and to use the recording in the production named on the form and in the media, territory and term stated. It must state whether the person will be credited and any fee. It does not grant use for other productions unless stated.

3.Explaining the release

The producer or director must explain the release in plain language, answer questions and allow the person time to read it. Where the person does not read English, an interpreter or translated summary must be provided.

4.Withdrawal

A person can withdraw consent for future uses by writing to the Operations and Finance Manager. Withdrawal does not affect material already published in line with the release.

5.Filing

The signed release must be scanned and filed against the job number in the Media Asset Library on the day of the shoot and the original kept in the production file.

6.Complete the form
